
Grant Applications Rules and Eligibility Requirements
The Heart Wishes Grant is open to legal citizens of the United States who are diagnosed with a critical health condition. Board members, Executive Director, and committee members of the National Association of Supplemental Benefit Insurance (NASBI), including members of their immediate families or persons living in the same household are not eligible to apply.
All submissions and all parts of the submission become the sole property of NASBI and will not be returned to the applicant.
Grants for applicants under the age of majority in the state of residence will be issued in the name of a parent or legal guardian who will be required to execute all necessary affidavits and releases.
Grant recipients must consent to the use of their name and likeness in promotional materials, including but not limited to Facebook, Instagram, and LinkedIn posts, without additional compensation and as stated in the signed Consent and Release.
Applicants’ submission must be entirely their own and must not infringe upon or violate any laws or any rights of third parties, including but not limited to such violations as infringement or copyright, patent, trademark, trade secret or other proprietary or property right, defamation, violation of rights of privacy, publicity, personality or celebrity, or any contract right, or any other right of any individual, corporation or entity. Applicants must obtain all necessary permissions, licenses, clearances, releases, waivers of moral rights, and other approvals from third parties (including but not limited to all copyright holders), necessary to use the submission, in whole or in part, in any way, including without limitation, to reproduce, make derivatives, edit, modify, translate, distribute, transmit, publish, license and broadcast worldwide, by any means. Any and all such permission, licenses, clearances, releases, waivers of moral rights and approvals must be provided to NASBI with the application.
Each applicant shall indemnify, defend, and hold NASBI harmless from any third-party claims arising from or related to that applicant’s participation in The Heart Wishes program. In no event shall NASBI be liable to an applicant or potential grant recipient for acts or omissions arising out of or related to the program or that applicant or potential grant recipient’s participation in the program. Participants agree that the sponsor and its respective affiliates, distributors, advertising and promotion agencies and all of their respective officers, directors, employees, representatives and agents shall have no liability and shall be held harmless for any damage, losses or injury resulting in whole or in part, directly or indirectly, from the acceptance, possession, use or misuse of the grant or participation in this process.
The decision of NASBI will be final and cannot be appealed. The odds of selection are based on the number of entries and the merit of the application.
All taxes, including income taxes, are the sole responsibility of grant recipients. No grant substitution is permitted. These rules and regulations are subject to change without notice for any reason, including without limitation, if necessary to comply with any applicable laws or regulations. By entering The Heart Wishes grant program, applicants and potential grant recipients agree to abide by The Heart Wishes Fund program rules and regulations and acknowledge that NASBI shall not be responsible for any damages, costs, demands, claims or losses of any kind, made in connection with, in respect of or arising out of The Heart Wishes Fund including without limitation, in connection with the development or use of the submissions.
By applying to this program, each applicant and potential grant recipient agrees to abide by these rules. The decisions of the judges will be binding in all respects. NASBI reserves the right to cancel, modify or suspend the program if fraud, technical failures, or other factors beyond NASBI’s control impair the integrity of the program, as determined by NASBI in its sole discretion.
